What forms of payment are taken?

We accept cash, check, credit card, healthy savings (HSA), or flexible savings accounts (FSA) for physical therapy services.

Can i bring my child with me?

Yes! Children are always welcome to join. Revive wants to create an atmosphere where mothers can get the care they need and deserve, and childcare shouldn’t get in the way of that! There is also a private nursing space available courtesy of Vera Chiropractic.

What do I wear to a visit?

A movement assessment will be performed, so wear something comfy and non-restrictive.

How do I find your clinic space?

We are located at 2716 Forum Blvd Suite 3. Revive Rehab is inside Vera Chiropractic on the second floor.

Do I need a referral for physical therapy?

No! Missouri is a direct-access state which means you are able to see a licensed physical therapist without a physician or provider referral/script. If the plan of care extends past 30 days, then a referral will be required. Your provider can fax the script to (855) 710-4225.

How do i submit a claim to my insurance for a possible out-of-network reimbursement?

You may be able to be reimbursed for physical therapy through your “out-of-network” benefits. Call the number on your insurance card and ask to speak with a representative regarding your “out-of-network” benefits for physical therapy (also called rehabilitation benefits). Make sure to ask if your insurance requires a special form to submit a claim for reimbursement. Revive Rehab can provide an itemized receipt with a list of services, if requested.
